Replens review: does Replens vaginal moisturizer work?

This Replens review covers Replens Long-Lasting Vaginal Moisturizer: an estrogen-free polycarbophil gel you apply every three days, giving up to 3 days of hydration per dose. Side effects are usually mild, mainly residue or discharge at first. In a 1997 trial in 45 breast cancer survivors it eased painful sex more than a placebo gel, although dryness improved almost as much with the placebo.

Replens Long-Lasting Vaginal Moisturizer is an estrogen-free polycarbophil gel used every three days. Ingredients, side effects, condom use and what the trials show.

Replens for vaginal dryness: expectation vs reality

What people often expect

“Replens is a lubricant you use right before sex”

What the evidence shows

Replens is a moisturizer: you use it every three days to keep the vagina moist, and its label says to apply it at least 2 hours before sex. During sex itself, a lubricant does the job a moisturizer can't.

Replens review: ingredients, how to use it and the evidence

Replens ingredients

Replens Long-Lasting Vaginal Moisturizer lists purified water, glycerin, mineral oil, polycarbophil, carbomer homopolymer type B, hydrogenated palm oil glyceride, methylparaben, sorbic acid and sodium hydroxide. Polycarbophil is the moisturizing polymer that trials have tested. The description says it's made without estrogen, parabens or fragrance, although the ingredient list on the same listing includes methylparaben, so check the box if you avoid parabens.

What causes the dryness Replens treats

Lower estrogen around menopause thins the vaginal lining, and NAMS estimates that vaginal and urinary symptoms of menopause affect 27% to 84% of postmenopausal women. Dryness can also come from breastfeeding, hormonal contraception, antidepressants, a hysterectomy, cancer treatment, perfumed soaps or douches, and conditions such as diabetes or Sjögren's syndrome. Replens eases the dryness itself, whatever the cause, but persistent symptoms need a check-up.

How to use Replens

Each pack has one reusable applicator and one tube with 14 applications. Use one application inside the vagina every three days, or as needed for everyday comfort, rather than only before sex. If you want it in place for sex, apply it at least 2 hours beforehand. The label notes you may see some residue or discharge when you first start using it.

Does Replens work? The trials

In a 1997 double-blind trial, 45 breast cancer survivors used Replens and a placebo gel for 4 weeks each. Dryness fell 64% with Replens and 62% with placebo, while painful sex improved 60% against 41%. In a 2022 trial of 53 women, polycarbophil gel worked as well as hyaluronic acid gel. A 2024 review found vaginal moisturizers may improve dryness, with low certainty.

Replens side effects

The label warns you may notice residue or discharge at first, and says to ask your healthcare provider before use if you're pregnant or breastfeeding. Both Replens and the placebo gel were well tolerated in the 1997 trial, and the 2022 polycarbophil trial reported no adverse events. Use only products made for the vagina: body creams and lotions can cause more irritation or an infection.

Replens and condoms

The Replens directions say it's compatible with natural rubber latex, polyisoprene and polyurethane condoms. The product description on the same listing says it's compatible with latex and polyisoprene condoms but not recommended with polyurethane ones. Until the label agrees with itself, latex or polyisoprene condoms are the safe pairing. That's a real advantage over Revaree, which Bonafide says isn't compatible with condoms.

Replens vs vaginal estrogen

NAMS says non-hormonal products available without a prescription give enough relief for most women with mild symptoms, while low-dose vaginal estrogen is effective for moderate to severe vaginal and urinary symptoms. A 2024 review rated the evidence for both vaginal estrogen and moisturizers as low certainty, mostly from trials of 12 weeks or less. If Replens isn't enough after a few weeks, ask about vaginal estrogen.

Replens vs Revaree and other moisturizers

Revaree, from Bonafide, is a hyaluronic acid insert used every 2 to 3 nights that isn't condom-compatible. In the 2022 trial, polycarbophil gel did as well as hyaluronic acid gel, and hyaluronic acid added no extra benefit. Experts advise choosing a moisturizer with a pH and osmolality close to natural vaginal fluid, which is worth asking a pharmacist about whichever brand you pick.

Replens after breast cancer

Replens is one of the few moisturizers tested specifically in breast cancer survivors: the 1997 trial found it and the placebo gel both substantially eased dryness and painful sex. Guidelines advise starting with non-hormonal options after breast cancer and discussing vaginal estrogen with your oncologist if they don't help. Most trials of vaginal treatments have included few women with a history of cancer.

Before you buy Replens

  1. Pick the Long-Lasting Vaginal Moisturizer: Replens also sells a Silky Smooth lubricant and an External Comfort Gel.
  2. If you use condoms, pair Replens with latex or polyisoprene ones; guidance on polyurethane condoms is mixed.
  3. If you avoid parabens, read the ingredient list on the box: the listing includes methylparaben.
  4. If you're pregnant or breastfeeding, check with your healthcare provider first.
  5. Use it every three days, not only before sex, and give it a few weeks.

When vaginal dryness needs a doctor, not Replens

Replens: questions women ask

Does Replens work?

It can help, but the evidence is modest. In a 1997 trial painful sex improved 60% with Replens against 41% with a placebo gel, while dryness improved about the same with both. A 2024 review found vaginal moisturizers may improve dryness, with low certainty.

What are the side effects of Replens?

The label says you may notice residue or discharge when you first use it. Trials found it well tolerated, and a 2022 polycarbophil trial reported no adverse events. If you're pregnant or breastfeeding, check with your healthcare provider before use.

What are the ingredients in Replens?

Purified water, glycerin, mineral oil, polycarbophil, carbomer, hydrogenated palm oil glyceride, methylparaben, sorbic acid and sodium hydroxide. It contains no estrogen or fragrance. The description says paraben-free, but the listed ingredients include methylparaben. Polycarbophil is the ingredient that has been tested in trials.

How do you use Replens?

Use one application inside the vagina with the reusable applicator every three days, or as needed for comfort. One tube holds 14 applications. If you want it in place for sex, apply it at least 2 hours before.

How long does Replens take to work?

In the 1997 trial, most of the benefit appeared within the first 2 weeks and then held steady. Use it regularly every three days for a few weeks before deciding whether it's enough for you.

Can you use Replens before sex?

Yes, but apply it at least 2 hours before, according to the label. Replens is a moisturizer rather than a lubricant, so many women also need a lubricant during sex itself. Moisturizers keep the vagina moist between times; lubricants ease friction during sex.

Is Replens safe with condoms?

With latex and polyisoprene condoms, yes, according to the label. The directions also list polyurethane condoms as compatible, but the description says they're not recommended, so stick to latex or polyisoprene. Revaree, by comparison, isn't compatible with condoms at all.

Replens or Revaree?

Replens is a polycarbophil gel that works with latex condoms; Revaree is a hyaluronic acid insert that doesn't work with condoms. In a 2022 trial of 53 women, polycarbophil gel worked as well as hyaluronic acid gel.

Can Replens help with itching?

It may, if the itching comes from dryness: soreness and itching in and around the vagina are common symptoms of vaginal dryness, and a moisturizer keeps the vagina moist. Itching with unusual discharge, a smell or sores needs a doctor or sexual health clinic to rule out infection.

Do you need a prescription for Replens?

No. Replens is sold in stores and online without a prescription. NAMS says non-hormonal products like it give enough relief for most women with mild symptoms; if dryness is moderate to severe or doesn't improve, prescription vaginal estrogen is more effective.
