Intermittent fasting in menopause
Intermittent fasting works for weight loss about as well as a regular calorie-controlled diet, not better. A 2026 Cochrane review of 22 trials found little to no difference in weight loss between fasting and standard dietary advice. In obese postmenopausal women, intermittent and continuous diets gave similar results over a year. Keto diets can cause slightly more weight loss than low-fat diets but raise LDL cholesterol. Neither is proven to ease hot flashes.

Intermittent fasting in menopause: expectation vs reality
What people often expect
“Intermittent fasting or keto is the secret to beating menopause weight gain”
What the evidence shows
Both are just ways to eat fewer calories. Trials show fasting matches, but doesn't beat, standard diets, and keto's small extra weight loss comes with higher LDL cholesterol, which already tends to rise at menopause. The best plan is the one you can keep up, combined with exercise.
Intermittent fasting and keto in menopause: what studies show
Intermittent fasting for weight loss
The 2026 Cochrane review included 22 randomized trials with 1,995 adults with overweight or obesity, covering time-restricted eating, alternate-day and periodic fasting. Compared with regular dietary advice, intermittent fasting made little to no difference in weight loss or quality of life. Most outcomes showed no clinically meaningful change between approaches.
Intermittent fasting for menopausal women
In a pilot trial, obese postmenopausal women followed either an intermittent diet (13 women), with 5-week restriction periods separated by weight-stable breaks, or a continuous diet (12 women). Weight, waist size and fat mass fell similarly in both groups, and at 1 year both kept weight off equally well. Menopause-specific fasting trials remain small.
Fasting and menopause symptoms
A 2026 systematic review of 15 diet trials in menopausal women found soy-based diets may modestly reduce hot flashes, with very low certainty. Evidence for intermittent fasting was limited to cognition. There's no good evidence that fasting reduces hot flashes, night sweats or mood symptoms.
Learn more →Keto diet for menopause weight
A meta-analysis of 13 trials found very-low-carbohydrate ketogenic diets led to slightly more long-term weight loss than low-fat diets, under a kilogram on average, with lower triglycerides and diastolic blood pressure. But LDL cholesterol rose. An umbrella review of 68 trials rated the LDL rise as high-quality evidence and clinically meaningful.
Keto, cholesterol and heart risk after menopause
Cholesterol tends to rise during menopause, and fat around the waist raises heart and diabetes risk. That makes keto's effect on LDL a particular concern. The best-supported eating patterns for heart and metabolic health through menopause are Mediterranean and DASH diets.
Learn more →Protecting muscle while cutting calories
Menopause speeds up muscle loss, and some keto diets reduced muscle mass in healthy people. Keep protein at 10% to 35% of calories and do muscle-strengthening exercise at least 2 days a week, whichever eating pattern you choose.

Intermittent fasting and keto in menopause: questions women ask
Does intermittent fasting work for menopause weight loss?
It works about as well as a standard calorie-controlled diet, not better. In obese postmenopausal women, intermittent and continuous diets gave similar results.
What is the best intermittent fasting for menopause?
No fasting schedule is proven best for menopause. Time-restricted eating, alternate-day and periodic fasting all performed similarly to regular diets.
Is intermittent fasting safe in perimenopause?
The Cochrane review found little to no difference in side effects vs regular diets, though evidence is very uncertain. Check with your doctor if you have diabetes.
Is keto good for menopause?
Keto may cause slightly more weight loss than low-fat diets, but it raises LDL cholesterol, which already tends to rise at menopause.
Does intermittent fasting help menopause belly fat?
In postmenopausal women, intermittent and continuous diets reduced waist size and fat mass similarly. Exercise specifically reduces visceral fat.
Can fasting help hot flashes?
There's no good evidence that it does. A 2026 review found evidence for intermittent fasting in menopause limited to cognition.
Is 16:8 intermittent fasting good for menopause?
16:8 is a form of time-restricted eating. In the Cochrane review, time-restricted and other fasting schedules gave similar weight loss to regular diet advice.
Does keto raise cholesterol in menopause?
It can. An umbrella review of 68 trials found keto diets caused a clinically meaningful rise in LDL cholesterol, backed by high-quality evidence.
Does keto cause muscle loss?
Some keto diets reduced muscle mass in healthy people, while very-low-calorie keto diets didn't worsen muscle mass in adults with obesity.
