Lynkuet (elinzanetant) for hot flashes

Lynkuet (elinzanetant) is a non-hormonal prescription capsule for moderate to severe hot flashes and night sweats due to menopause, approved by the FDA in 2025. The dose is two 60 mg capsules once a day at bedtime. In trials it cut hot flashes by about 3 a day more than placebo from week 4. It can cause sleepiness, needs liver blood tests before starting and at 3 months, and must not be taken in pregnancy.

Lynkuet for hot flashes: expectation vs reality

What people often expect

“Lynkuet is a sleeping pill for hot flashes, or a hormone-free drug that works as well as HRT with no downsides”

What the evidence shows

It helps, but modestly. In OASIS 1 and 2, women started with 13.4 and 14.7 moderate to severe hot flashes a day. At week 12, they had 8.66 and 9.72 fewer on Lynkuet, against 5.44 and 6.48 fewer on placebo. Sleepiness, fatigue, dizziness or similar effects hit 11.9% of women on Lynkuet and 3.5% on placebo, and in 2026 the label added a seizure warning.

How Lynkuet (elinzanetant) works and what to expect

How elinzanetant works

Elinzanetant blocks two receptors, neurokinin 1 and neurokinin 3, on KNDy neurons in the brain's temperature control center. When estrogen falls at menopause, signaling through neurokinin B and substance P on these neurons helps trigger hot flashes. Lynkuet isn't a hormone and doesn't replace estrogen, so it doesn't treat vaginal dryness. Veozah, the first drug of this kind, blocks only the neurokinin 3 receptor.

How well Lynkuet works for hot flashes

In OASIS 1 and 2, 796 postmenopausal women aged 40 to 65 with at least 50 moderate to severe hot flashes a week took Lynkuet or placebo. Lynkuet cut hot flashes by 3.3 and 3.0 a day more than placebo at week 4, and by 3.2 at week 12 in both trials. Hot flashes also got milder, and sleep disturbance and menopause-related quality of life improved by week 12.

Lynkuet over a full year: OASIS 3

OASIS 3 followed 628 postmenopausal women for 52 weeks, with no minimum number of hot flashes needed to join. At week 12, daily moderate to severe hot flashes fell by 5.4 on Lynkuet and 3.5 on placebo, a difference of 1.6. The researchers found no liver toxicity, no endometrial hyperplasia signal and no meaningful change in bone density over the year. Treatment-related side effects were more common with Lynkuet: 30.4% vs 14.6%.

Lynkuet and sleepiness: why it's taken at bedtime

Lynkuet slows down the nervous system in some women. In the three OASIS trials, sleepiness, fatigue, vertigo, dizziness or feeling faint affected 11.9% of women on Lynkuet and 3.5% on placebo. In a driving simulator study 9 hours after a bedtime dose, the average woman wasn't impaired, but some were after the first dose, and fewer after 5 days. Don't drive until you know how it affects you.

Lynkuet and the liver

You need liver blood tests (ALT, AST, alkaline phosphatase and bilirubin) before starting, and Lynkuet shouldn't be started if liver enzymes or bilirubin are 2 or more times the upper limit of normal. Enzymes are checked again at 3 months. In trials, enzymes rose above 3 times normal in 0.6% of women on Lynkuet and 0.4% on placebo. Unlike Veozah, Lynkuet has no boxed warning for liver injury.

The Lynkuet seizure warning

In August 2026, the FDA label added a warning about seizures after postmarketing reports. In the clinical trials, one woman (less than 0.1%) with a history of seizures had one. Rats given elinzanetant at 4 times or more the human exposure had convulsions. The label advises caution if you've had seizures or have a condition that lowers the seizure threshold, so tell your prescriber about either before starting.

Lynkuet, perimenopause and pregnancy

Lynkuet must not be taken in pregnancy: in animal studies it caused pregnancy loss and stillbirth. Pregnancy should be ruled out before starting, and women who could still conceive need effective contraception during treatment and for 2 weeks after stopping. This matters in perimenopause, when periods are irregular but ovulation still happens. The trials enrolled only postmenopausal women aged 40 to 65, so there's no trial data in perimenopause.

Lynkuet drug interactions and grapefruit

Elinzanetant is broken down by the liver enzyme CYP3A4. Strong CYP3A4 blockers, such as the antifungal itraconazole, raised its levels 6.3-fold, so they and grapefruit juice must be avoided. With moderate CYP3A4 blockers, the dose drops to one 60 mg capsule. Strong and moderate CYP3A4 inducers, such as carbamazepine, cut its levels by 64% and should be avoided. Tamoxifen levels weren't meaningfully changed.

Lynkuet after breast cancer: the OASIS 4 trial

OASIS 4 tested Lynkuet in 474 women aged 18 to 70 with hot flashes caused by endocrine therapy for hormone receptor-positive breast cancer or its prevention. From about 11.4 hot flashes a day, women on Lynkuet had 7.8 fewer at week 12, against 4.2 fewer on placebo. Serious side effects in the first 12 weeks occurred in 2.5% vs 0.6%. Agree any treatment with your oncology team.

How to take Lynkuet

Take two 60 mg capsules once a day at bedtime, at about the same time each night, with water, with or without food. Swallow them whole; don't cut, crush or chew them. If you miss a bedtime dose, skip it and take the next one the following night; never take two doses in one day. With a half-life of about 45 hours, levels steady after 5 to 14 days.

Lynkuet side effects over 52 weeks

In the one-year OASIS 3 trial, side effects more common than on placebo were headache (9.6% vs 7.0%), fatigue (7.3% vs 2.9%), dizziness (6.1% vs 1.9%), sleepiness (5.1% vs 1.3%), abdominal pain (4.5% vs 2.5%), rash (4.2% vs 1.6%), diarrhea (3.8% vs 1.0%) and muscle spasms (3.2% vs 0.6%). Mild sun sensitivity occurred in 0.5%. Endometrial biopsies found no cancers.

Lynkuet cost, savings card and insurance

Lynkuet is brand-only, with no generic. The cash price is around $618.40 for 60 capsules, a 30-day supply, depending on the pharmacy. With commercial insurance, the maker's copay card can bring it down to as little as $25 a month. Uninsured women with income at or below 300% of the federal poverty level can apply to the Bayer US Patient Assistance Foundation. Ask your plan about prior authorization.

Lynkuet vs other hot flash treatments

  • Lynkuet (elinzanetant)

    Strong evidence

A bedtime capsule that targets the brain signal behind hot flashes and, in trials, also improved sleep disturbance. Needs liver tests before starting and at 3 months, can cause next-day sleepiness, and can't be used in pregnancy. A good option if you can't take hormones and night sweats break your sleep.

  • Veozah (fezolinetant)

    Strong evidence

The first neurokinin blocker: one 45 mg tablet a day at any time, no sleepiness warning. It has a boxed warning for rare serious liver injury, with tests before starting and at months 1, 2, 3, 6 and 9, and it can't be taken with CYP1A2 blockers such as fluvoxamine.

  • Estrogen HRT

    Strong evidence

More effective than any non-hormonal option, cutting hot flash frequency by about 75%, and it also treats vaginal dryness and protects bone. It isn't suitable after hormone-sensitive breast cancer or with some clot and liver conditions, which is exactly where Lynkuet and Veozah come in. If hormones are open to you, ask about them first.

  • SSRIs, SNRIs and gabapentin

    Strong evidence

Older non-hormonal options that cut hot flashes by about 40% to 65%. They're cheap generics with no liver testing requirement, which matters if insurance won't cover Lynkuet. Gabapentin taken at night can also help sleep. Paroxetine is avoided with tamoxifen because it can weaken it.

When to call a doctor on Lynkuet

  • Urgent: A seizure or passing out on Lynkuet

Call 911 or get emergency care right away, and tell the team you take elinzanetant.

  • Urgent: Yellow skin or eyes, dark urine, pale stools, itching, nausea or new tiredness

Stop Lynkuet and contact your prescriber the same day for liver blood tests. These can be signs of liver injury.

  • Urgent: You might be pregnant

Stop Lynkuet and tell your prescriber right away; it can cause pregnancy loss.

  • Sleepiness or dizziness the next day

Don't drive until it passes, and ask your prescriber whether another medicine you take is raising your Lynkuet levels.

  • Starting an antifungal, antibiotic or any new medicine

Check with a pharmacist first: many interact with Lynkuet through CYP3A4.

  • Any bleeding after menopause

See a doctor; bleeding always needs checking, whatever treatment you're on.

Lynkuet: questions women ask

What is Lynkuet used for?

Lynkuet treats moderate to severe hot flashes and night sweats caused by menopause. It's a non-hormonal capsule that works on the brain's temperature control center, taken as two capsules at bedtime. It doesn't treat vaginal dryness and isn't a sleeping pill, though trials showed better sleep as hot flashes eased.

How well does Lynkuet work?

In OASIS 1 and 2, it cut moderate to severe hot flashes by about 3 a day more than placebo by week 4, and by 3.2 a day more at week 12, from a starting point of 13.4 to 14.7 a day. Estrogen HRT is more effective, cutting hot flash frequency by about 75%.

What are the side effects of Lynkuet?

The most common are headache, fatigue, dizziness and sleepiness, each in 5% or more of women. Abdominal pain, rash, diarrhea and muscle spasms are less common. Rarer but serious: raised liver enzymes and seizures. Stop and call your prescriber if you notice yellow skin or eyes, dark urine or unusual tiredness.

Does Lynkuet make you sleepy?

It can. Sleepiness, fatigue or dizziness affected 11.9% of women in trials, compared with 3.5% on placebo, which is why it's taken at bedtime. Some women's driving was impaired the morning after the first dose. If you feel drowsy, don't drive until the effect has worn off.

How long does Lynkuet take to work?

In trials, hot flashes fell more than on placebo by week 4, the first checkpoint, and the effect held at week 12. Blood levels steady after 5 to 14 days. Give it about 12 weeks before judging whether it works for you.

How much does Lynkuet cost?

Around $618.40 for 60 capsules, a 30-day supply, if you pay cash, depending on the pharmacy. There's no generic. With commercial insurance, the maker's copay card can bring it to as little as $25 a month. Uninsured women on lower incomes can apply to the Bayer patient assistance program.

Does Lynkuet cause weight gain?

Weight gain isn't listed among Lynkuet's common side effects in the label or the trials. Weight often changes around menopause for other reasons, including muscle loss and shifts in fat distribution, so look at diet, strength training and sleep if the scale moves.

Is Lynkuet safe for the liver?

Liver enzymes rose above 3 times normal in 0.6% of women on Lynkuet and 0.4% on placebo, and it has no boxed warning, unlike Veozah. You still need liver tests before starting and at 3 months, and you should stop it if you notice signs of liver problems.

Can I eat grapefruit or take other medicines with Lynkuet?

Avoid grapefruit and grapefruit juice: they raise elinzanetant levels. So do strong CYP3A4 blockers such as itraconazole, which must be avoided, and moderate ones, which halve the dose. Carbamazepine and other inducers make it less effective. Show your pharmacist every medicine and supplement you take.

Can I take Lynkuet after breast cancer or with tamoxifen?

The OASIS 4 trial tested it in women with hot flashes from endocrine therapy for breast cancer, and it cut hot flashes by 3.4 a day more than placebo at week 12. Tamoxifen levels weren't meaningfully changed by elinzanetant. Agree the choice with your oncology team.

Lynkuet vs Veozah: which is better?

No trial has compared them directly. Lynkuet is taken at bedtime and can cause sleepiness; Veozah is one tablet at any time but has a boxed liver warning and more liver tests. Interactions differ too: CYP3A4 for Lynkuet, CYP1A2 for Veozah. See the full comparison.
