FSH levels in perimenopause and menopause

The FSH value in menopause is high because the pituitary keeps pushing ovaries that no longer respond. Labcorp's postmenopausal range is 25.8 to 134.8 mIU/mL, compared with 3.5 to 12.5 in the follicular phase. In perimenopause, FSH swings: an expert staging system says a random level above 25 IU/L is characteristic of the late transition, but it can drop back to normal. From 45, NICE doesn't use FSH to diagnose menopause.

FSH rises as the ovaries slow, but in perimenopause it swings day to day. Ranges by stage, why one FSH test can't confirm perimenopause, and when it's useful.

FSH levels in menopause: expectation vs reality

What people often expect

“A high FSH test proves I'm in menopause, and a normal one proves I'm not”

What the evidence shows

In the late menopause transition, FSH is sometimes in the menopausal range and sometimes back in the reproductive range, especially when estradiol is high that day. One normal result doesn't rule out perimenopause, and one high result isn't menopause. That's why NICE uses FSH only to help confirm menopause between 40 and 45, or under 40.

FSH levels in perimenopause and menopause explained

What FSH does

Follicle-stimulating hormone is made by the pituitary gland at the base of the brain. In women who have periods, it makes eggs grow in the ovaries and prepares them for ovulation. The ovaries' hormones feed back to the brain: when the ovaries make fewer hormones, the pituitary makes more FSH to try to get the ovaries working. That feedback is why FSH climbs as the ovaries wind down.

FSH levels chart: cycle phases and menopause

Labcorp's adult female FSH ranges are 3.5 to 12.5 mIU/mL in the follicular phase, 4.7 to 21.5 at ovulation, 1.7 to 7.7 in the luteal phase and 25.8 to 134.8 after menopause. For LH, the matching ranges are 2.4 to 12.6, 14.0 to 95.6, 1.0 to 11.4 and 7.7 to 58.5 mIU/mL. Other labs use slightly different ranges.

FSH levels in perimenopause

In the early transition, FSH early in the cycle is raised but variable. In the late transition, marked by gaps of 60 days or more between periods, a random FSH above 25 IU/L is characteristic, according to STRAW+10, but FSH can return to the reproductive range when estradiol is high. In one study, FSH, LH and estradiol all rose with each stage of reproductive aging.

When FSH starts to rise before menopause

In SWAN, which followed 1,215 women to their final period, FSH began rising about 6 years before the last period, accelerated about 2 years before it and leveled off about 2 years after. Obesity blunted the rise and delayed its start. Women followed low (10.6%), medium (48.7%) or high (41.7%) FSH patterns, so there's no single curve.

What level of FSH indicates menopause?

After menopause, Labcorp's FSH range is 25.8 to 134.8 mIU/mL, and STRAW+10 uses above 25 IU/L as characteristic of the late transition. But no single level diagnoses menopause: it's confirmed after 12 months without a period. NICE says to consider FSH to confirm menopause only between 40 and 45 with symptoms and cycle changes, or under 40 when menopause is suspected.

FSH and estradiol together

Doctors read FSH alongside estradiol and LH. High FSH with low estradiol points to ovaries that are winding down or failing, as in menopause or primary ovarian insufficiency. Low FSH with low estradiol points to the pituitary or hypothalamus, as with rapid weight loss, being very underweight or extreme exercise. High estradiol on the day can pull FSH back into the normal range.

When not to test FSH

NICE says not to use FSH to identify menopause in women taking combined hormonal contraception or high-dose progestogens. It also says not to use estradiol, AMH or inhibin tests to identify perimenopause or menopause from 45. NICE also notes that menopause can be hard to identify in women taking hormonal treatments, for example for heavy periods.

Other causes of high FSH

Besides menopause and perimenopause, high FSH in women can come from primary ovarian insufficiency, ovaries that never developed normally, PCOS, ovarian tumors, thyroid or adrenal disorders, Turner syndrome, and ovarian damage from autoimmune disease, radiation or chemotherapy. That's why a high FSH before 40 needs a proper workup.

When an FSH test is useful

  • FSH test between 40 and 45

    Strong evidence

With menopause symptoms and cycle changes, an FSH level can help confirm menopause, and NICE supports its use in this group.

  • FSH test under 40

    First step

When menopause is suspected before 40, FSH testing is part of diagnosing primary ovarian insufficiency, which needs treatment to protect bones and heart.

  • Symptoms and cycle from 45

    First step

From 45, perimenopause is identified from recently started hot flashes and any cycle change, and menopause after 12 months without a period, no FSH needed.

  • Home FSH urine kits

    Limited evidence

They detect raised FSH, but because FSH swings in perimenopause, a positive or negative result doesn't settle whether you're in the transition.

When to ask a doctor about FSH testing

  • Periods stop or become irregular before 40

See a doctor. FSH tests help diagnose primary ovarian insufficiency.

  • Symptoms and cycle changes between 40 and 45

Ask whether an FSH test would help confirm early menopause.

  • Trying to get pregnant in your late 30s or 40s

See a doctor; FSH is part of checking ovarian reserve and the cause of infertility.

  • Missed periods with rapid weight loss or heavy exercise

See a doctor. Low FSH and estrogen can point to hypothalamic causes.

FSH and menopause: questions women ask

What FSH level indicates menopause?

After menopause, Labcorp's FSH range is 25.8 to 134.8 mIU/mL, and a random level above 25 IU/L is characteristic of the late transition. Menopause itself is confirmed after 12 months without a period, not by one FSH result.

What are FSH levels in perimenopause?

They're raised but variable. Early in the transition, FSH early in the cycle is higher than before; in the late transition it's often above 25 IU/L but can fall back to normal, especially when estradiol is high.

What are normal FSH and LH levels after menopause?

Labcorp's postmenopausal ranges are 25.8 to 134.8 mIU/mL for FSH and 7.7 to 58.5 mIU/mL for LH. Both rise because the ovaries no longer respond.

Is FSH testing for menopause accurate?

Not on its own in perimenopause, because FSH swings from cycle to cycle. NICE uses it only between 40 and 45, or under 40, and says not to use it with combined hormonal contraception or high-dose progestogens.

What is the difference between FSH and estradiol?

FSH is the pituitary's signal to the ovaries; estradiol is the estrogen the ovaries make in response. As the ovaries wind down, estradiol falls and FSH rises, so doctors read them together.

Can FSH be normal in perimenopause?

Yes. Even in the late transition, FSH can be in the reproductive range on some days, particularly when estradiol is high. A normal FSH doesn't rule out perimenopause.

What does low FSH mean in women?

Low FSH can point to a problem with the pituitary gland or hypothalamus, or be linked to rapid weight loss, being very underweight or extreme exercise. It's also low in the luteal phase of a normal cycle.

Do home FSH tests work for menopause?

They measure FSH in urine, but FSH varies so much in perimenopause that one result can mislead. Your age, periods and symptoms are more reliable guides.

Related →

What is FSH testing used for in women?

To help find the cause of infertility or irregular and stopped periods, to check how the ovaries are working, and to help show whether perimenopause or menopause has begun, mainly in women under 45.

How long before menopause does FSH rise?

In SWAN, FSH began rising about 6 years before the final period, sped up about 2 years before it and stabilized about 2 years after. Obesity delayed and blunted the rise.

What FSH value means perimenopause?

There isn't a single value. STRAW+10 describes FSH above 25 IU/L on a random blood draw as characteristic of the late menopause transition, but FSH can fall back to the reproductive range, especially when estradiol is high. Your periods and symptoms matter more.
